Applications Now Available for TennCare’s Patient-Centered Medical Home

In partnership with TennCare’s three managed care organizations, the Tennessee Division of Health Care Finance and Administration (HCFA) launched a statewide aligned patient-centered medical homes (PCMH) program with 29 organizations on Jan. 1.

PCMH is a comprehensive delivery care model designed to improve:

  • The quality of primary care services for TennCare members.
  • The capabilities and practice standards of primary care providers.
  • The overall value of health care delivered to the TennCare population.

Participating organizations commit to member-centered access, team-based care, population health management, care management support, care coordination, performance measurement and quality improvement.

Applications now are being accepted for additional PCMH organizations to join the program. Participation for new providers will begin Jan. 1, 2018. The application may be found here and information about the program may be found here. Applications are due by 5 p.m. CST on June 30.
